Empowering Communities for DR-TB: Advancing Awareness and Access to BPaL/M Treatment

Bangladesh has a high burden of Drug-Resistant Tuberculosis (DR-TB), ranking 7th globally in overall TB burden and 14th among countries with high MDR-TB prevalence. The country has an estimated 15782 people affected by Drug-Resistant Tuberculosis (DRTB), with a significant portion of these patients requiring more effective and manageable treatment options. Despite the introduction of the World Health Organization (WHO) endorsed BPaL/M regimen, which offers a breakthrough 6-month, all-oral treatment for DR-TB, the rollout in Bangladesh has faced several hurdles. However, the adoption and scale-up of BPaL/M in Bangladesh face several challenges- these include limited awareness and understanding of BPaL/M among healthcare providers and the general population, logistical constraints in implementing widespread testing and treatment, and potential stigma associated with TB preventive therapy. Nari Maitree, with its extensive expertise in TB control, community mobilization, and digital health solutions, is well-positioned to lead efforts in addressing these gaps. By empowering communities, equipping healthcare workers, and supporting the National TB Program (NTP), Nari Maitree aims to accelerate the deployment of the BPaL/M regimen across Bangladesh and improve the overall well-being of DR-TB patients.

Empowering Communities for DR-TB: Advancing Awareness and Access to BPaL/M Treatment

Goal of the project:

To improve the lives of TB-affected communities by increasing awareness of and access to BPaL/M treatment, fostering community engagement, and strengthening collaboration with the NTP.

Objective of the project:

  • Community led demand creation by increasing awareness of BPaL/M among TB-affected communities, healthcare providers, and key stakeholders;
  • To strengthen community understanding of drug-resistant TB, especially the newly adopted BPaLM regimen;
  • Facilitate community-led engagement and monitoring in the TB treatment response to support the rapid and effective deployment of BPaL/M;
  • Capacitate TB-affected communities to participate meaningfully with the National TB Program (NTP) in facilitating the rollout of new treatments.

Major activities based on the planned outputs:

  • Activity 1.1: Establish and train a network of 30 BPaL/M ambassadors
  • Activity 1.2: Conduct baseline and endline KAP assessments to evaluate changes in TB knowledge, attitudes, and practices
  • Activity 1.3: Develop and disseminate culturally appropriate IEC materials on BPaL/M
  • Activity 1.4: Facilitate knowledge transfer and capacity building for 25 NTP staff to support BPaL/M implementation
  • Activity 2.1: Launch a multimedia awareness campaign, integrating digital content and collaboration with FTTC partners
  • Activity 2.2: Organize 3 high-level advocacy workshops and meetings with key stakeholders
  • Activity 3.1: Conduct 10 community based awareness, training and capacity building sessions for strengthening community led demand generation.
  • Activity 3.2: Establish 4 support groups.
    Activity 3.3: Establish community feedback mechanisms.
  • Activity 4.1: Establish a community driven monitoring system.
  • Activity 4.2: Collect and analyze data on BPaL/M uptake.
  • Activity 5.1: Jointly develop a sustainability plan.
  • Activity 5.2: Develop a final project closure report.
